Understanding CFSS

What is CFSS?

Community First Services and Supports (CFSS) is a Minnesota health care program that offers flexible options to help people live more independently at home and in their communities. It replaced PCA and the Consumer Support Grant and is intended to give people more choice and control over how their support is arranged.

Who may use CFSS?

Eligibility depends on an assessment and Minnesota program requirements. EvrBranch can help you understand the general process, but only an authorized agency can determine eligibility. To request an assessment, contact your county or tribal nation; if you are age 65 or older and enrolled in an MCO, contact your health plan. You can also contact us, and we can help direct you.

What choices does CFSS provide?

CFSS includes different ways to arrange and manage support. The right approach depends on your situation, preferences, and approved services. CFSS offers an agency model and a budget model. In the agency model, a provider agency employs and manages workers; in the budget model, you are the employer and receive support from a Financial Management Services provider.

How does CFSS work?

After assessment and eligibility steps, you choose a consultation services provider, learn about your options, and develop a Service Delivery Plan. Consultation Services supports you through the planning process and your lead agency approves the plan and authorizes services before they can begin.

What is a Service Delivery Plan?

Your Service Delivery Plan describes how you want to use approved CFSS services to support your needs, preferences, goals, and daily life. It can include help with daily activities (ADLs), household and community tasks (IADLs), health-related tasks, goods and services, emergency and backup plans, and worker support.

What does a Consultation Services provider do?

A Consultation Services provider explains CFSS options, helps you develop and review your Service Delivery Plan, and provides ongoing support as needed. They provide neutral information so you can make your own informed choices.

What can I expect?

You can expect clear explanations, room for questions, and support centered on your goals and choices. You do not need to understand every part of CFSS before the first conversation.
